Pdf

anyone here familar with pdf? i'm using adobe acrobat 7.0 pro n wanna create a blank pdf like below. any help is appreciated. thanks.

Thanks for the info, my problem is not knowing how to shrink the content in the box so i can edit on the borders?

you cant, thaTs why make your word doc the size u wanT then save as pDF.

noW if u had ILLUstrator or pHotoshop... tHen u can reSize the pDF

for some reason when it gets get converted from word to pdf, the image/text gets degraded n distorted a bit.

thanks ming. i guess i should explain further.
below i have 2 pdf files. however, "page1fromword" was create from how inkblue suggested. as u can see, the words and numbers dont look exactly the same. some lines r not even striaght.
its there a way of replicating "page1" into another pdf with the exception of it in a smaller scale but not degrade and deform the orginal. i'm assuming the degrade n deform is due to the conversion (from pdf-word-pdf).
