I currently have a PBT in a quarantine tank being treated for ich with hyposalinity. The SG is between 1.007 and 1.008 and is tested with a refractometer. After 21 ich free days, he has devloped several new spots of ich. I have read about there possibly being hyposalinity resisitant ich and have read about the "transfer method" which I think will be too much for us to do.
I'm looking for any other suggestions or thoughts.

The hyposalinity treatment along with Garlic Xtreme will do the trick. Some drops in the food might help, although I don't know if the allicin in the garlic actually gets into the body this way.

If you have a saline resistent strain of ich I would recommend slowly raising the salinity and changing to copper - cupramine would probably be the least harsh copper treatment on the tang.

You can run copper with hypo but remember to keep a close eye on the PH - lower PH will increase the toxity of copper.

Ouch :x They still make copper treatment? Just kidding.

You wouldn't want to risk using copper treatment on a powder blue. It's important to allow the fish to build its own immunity. Copper and hypo just seem too stressful.

Thanks for the feedback. I have recently added garlic to his food once a day, hoping that it would help. Should it be an "everyday" addition?
I guess this also buys our PBT another month in the quarantine.....
I'll keep everyone updated.

If the ich has resurfaced after 21 days at 1.007 then keeping it in hypo for a longer period of time will be unlikely to have any effect on the ich. Additionally, while there has been some antidotal evidence that garlic may help with ich I sure would not use it as a front line treatment.

Raise your salinity and treat with cupramine.

Great! I'm comfortable with both soaking food and doses in the tank daily. Garlic is completely reefsafe. Personally, I prefer just garlic treatments in the main tank as opposed to added stress of hypo or FW dip treatment.

There are a lot of skeptics when it comes to garlic, but the few times I've dealt with ich, I've had 100% success with just garlic alone. Even with sensitive fish. Before garlic products were available, my Moorish Idol was stricken with ich. I just cured him by squeezing garlic cloves in the water and feeding him chopped up bits.

The efficacy of garlic seems to lie in the antibiotic properties of the allacin. I would have to agree with Kevin here, with the caveat that formalin could be used instead of the copper.

I have performed hypo many times and also used garlic (GarlicX). Great combination. If your fish are not wanting to eat garlic will help entice him to eat. I have noticed that fish seem to heal quicker with garlic then without.

I think you are jumping the gun. You havent had him in hypo long enough:

Hyposalinity should be maintained for at least 4 weeks but 6 weeks is preferable. If there is any reinfection of the "Ich" during the treatment, the treatment should be extended to at least 4 weeks after the disappearance of the last cyst.

It is normal for the ich to come back sometime later during the hyposalinity treatment sometimes. No worries. Keep going with the hypo. If you havent found a good guidline for hypo yet please read this brutha!

i know everyone will hate me for doing this, but i successfully treated my ich by qting all my fish... and treating with CLOUT... 2 treatments i followed the directions then i did hyposalinity for about 2 weeks. i was going to wait longer but i could not control the ammonia levels in my qt tank so i decided to go against what everyone had recomended and put them back in the main tank, now nearly 8 months later and no signs of ich, not saying i recomend just , just trying to give you some hope.
