he has small spots of discoloration on him ,not ich tho i know what that looks like also his dorsal fin has some discolored areas between a few of the spines
and i also noticed a tear on its tail fin

when i noticed the fishs' condition i checked the tank temp and it was a tad high so im thinking that this could be a stress reaction the water was at 84f

Tangs are notoriously subsceptible to trematodes. Often these will cause discoloration and what look like little scratches on the fish. Prazi-pro will treat this effectively. If this is a reef tank, definitely remove the fish and treat it in a seperate quarantine tank. There is no guarantee that the other fish won't carry the parasite and cause the tang to be re-infected when you put it back in. You may also want to try a 5 minute freshwater bath with 1 drop of formalin per gallon. This is helpful with a lot of diseases. I do this with all new fish that I get. This is also the method that the NC Aquarium uses on all new fish.
