How long have you had it for? Have you tried moving it to a different location in the tank?

Had it for a couple of weeks and yeah jus moved it to a different location and its still happening

IME, Don't move it around too much as you can stress it out more.
I'm sure you'll be asked to post your water parameters and what sort of lighting you have. If you provide that info, it will be easier to identify any problems.
Also how did it look before you bought it? What kind of lights was it under in the store?
