What are your biggest fishkeeping "oops!"?

I regenerated some Purigen and thought I rinsed it out enough....literally within the hour LPS shriveled, SPS started RTNing soem fish labored breathing........a few water and carbon changes and things went back to normal......BTW to regenerate Purigen is with bleach..:skull:
