It might be dead then... your tank is way too new to have a fromia in it. They have a very restricted diet and if it was wandering around your tank in search of food it was likely starving :(
They only eat 'film' algae and that is something only found in well established tanks.

Definitely looks like thr Fromia is dead. Being your tank is only 3 months old, My guess would be the scooter blenny will be next.
There are a ton of critters in this hobby that need WELL established tanks to survive. You have to do your research before buying livestock so you know what the requirements are for the critter you want to buy.

+1 hes right. I had very similar advice given to me not to long ago. (tanks been up for 9months or so) its trial and error.
