What size is your QT? If it's a small QT Check your levels in there. Maybe having two big fish in there could've raised ammonia and caused some type of health problem to the purple tang. Don't see why he would die just like that during acclimation. That sucks to lose a purple that big.

I've had fish/ inverts die suddenly like that due to drastic pH change.
1. freshwater cherry shrimps, which were happy in the bag, then I dumped them into the bucket I used for saltwater changes. Dead in seconds.
2. uncured hydraulic cement mix (building reef rock) placed in tank, pH spiked within minutes and all my fish sunk to the bottom with rapid breathing. Some never regain buoyancy.
3. First time I set up a saltwater tank, noob mistake, added salt directly to the main tank since salinity was low. Just mixing it in the sump... sailfin tang in main tank died within a minute.

OH, There are a few species of African cichlids that also shock and die from this heart attack (fear?) and finally the bait fish spearing that I net, seem to die as soon as I catch them out of the net... freshly netted then as I throw them back into the water, they are just floating and not breathing.
