I realize from time to time we all lose fish, for a variety of reasons and this certainly is not the first fish i have lost.
I am concerned though as the fish (foxface approx. 5.5") was doing great. I would say I have had the fish for at least 5-6 months. It ate last night as normal but I noticed the other day it was not picking on the algae sheets like it usually would. I cant say i noticed anything on the fish for identification purposes for the cause of death. It was stuck to a powerhead this morning and quickly removed from the tank. I will do a water change later and change the carbon when I get home, hoping for a random death and not something brewing in the tank.

Wouldnt that be F.C.S.I.?

I had a similar event with my yellow coris wrasse, but I think my coral banded shrimp may have got to him...he had a bug chunk missing from his side but not sure if before or after. Too bad I flushed the evidence.. Does that make me an accomplice?
