So the last fish inhabitant of my aquarium is about to pass on and after taking a full battery of water parameter test and going with everyone's suggestions on what I thought was a Bristle Worm problem I think the following photographs below would most likely point to some type of water born disease except I'm not expert on marine disease so if you have any thoughts than please chime in. It doesn't seem to affect any of the inverts, clams or corals within the aquarium. Your assistance in advance is greatly appreciated.

are you sure your parameters are ok it kinda looks like when the slime coat is gone,I seen it happen when ammonia is too high the slime coat melts off and soon after the fish passes,looks like the skin is irritated.Change some water at least.

I see you have an 8 gallon aquarium. and from what I see you have one fish in the tank? Also , I see what looks like dinoflagellate on your rocks? As far as having good parameters , can u post some?

Yes, the parameter are fine. I tested multiple time just to be sure and even had my wife pick up a second set of test kits just to be certain. It appears to be what a few other people and my own research call Brooknella.

It's actually a 12 gallon now due to a recent upgrade. Please see the parameters below. Nitrate was at 25 ppm a week ago. Now here are no fish in the aquarium.

Salinity Grade: 1.025 SG
Temperature: 78.0 Fahrenheit
Ammonia: 0 ppm
Nitrite: 0 ppm
Nitrate: 5 ppm
pH: 8.0 pH
Alkalinity: 11.18 dKH
Magnesium: 1485 ppm
Calcium: 400 ppm
Phosphate: 1 ppm

Its Brooklynella and very common with clownfishes and it takes over the fish very fast. When ever i had this issue i took the fish out and put it in your QT if you have one going. Good luch i hope she makes it....
