John, I think you might be misreading the title. I think it might mean the before picture is prior to them taking any drug. The 'after' picture I think means after they've been using methamphetamine (crystal meth), not using methadone to break the heroin addiction. The 'before' pictures the people look totally normal.

FYI, this is info that came from my brother-in-law, a pharmacist. It might not be 100% correct, but it puts another spin on your view.

I was meaning methadone to break the heroin addiction. I see alot of that and I just think it just don't work IMO. Either way it's sad
