I have used ATI 36" 6 Bulb T5HO fixture. Comes with hanging kit to mount from your ceiling with aircraft cable.
Fixture works perfectly,, I just changed out all the lamp sockets for new ones. One of the fans needs to be replaced since I accidentally ripped out one of the wires but other fan work perfectly. You can buy the fan from ATI directly.
