Sam I feed prime reef flakes everyday, about 20 nickle sized flakes. I have 12 fish, anthias, hawk, chromis, cardnials, 7" hippo, 6" foxface.

Is the skimmer holding a foam out of the top of the neck in the collection cup? try leaving everything as dialed but clean the upper half of the skimmer. It skimms better when clean.

Hay Rich you got a nice mix of different fish there. My hippo is only like 2.5 inches and the yellow tank is like 5 inches, the rest are small. So i guess that's not a large bio load for them in like 100gallon water. As for the skimmer, i don't see much going on, just some foam collecting in the middle of the skimmer, pretty much nothing skim out. I'll clean it out when i get home, and hopefully it'll kick in soon. Thanks

Nitrate of 40 would not cause the problem. A couple years ago I had nitrates of about 50 for a year I never lost anything, it sounds like you are having some other problem that would be causing the deaths. keep looking. also try doing several 50% water changes over the course of 2 weeks. you should see a nice drop in Nitrates.
by the 3rd change you should be at about 5 nitrates.
